ATHLETICS FORMS: Sports Emergency Form & Athletic Contract

Completed and signed Sports Emergency Forms and Athletic Contracts are mandatory for participation in the school athletic program.  If the school does not receive this information, your child will not participate.  Copies of the information will be on file at school and will be carried by the coaches to all games. Please complete and return the following forms (Sports Emergency Form and Athletic Contract) to the school office. Please take extra care to include insurance policy numbers on the emergency form and have appropriate parental and student signatures as requested.  Incomplete forms will be mailed back to you for completion.
